Not a psychiatrist, but I worked in a psych ward. We had a young guy, around 19–23, who was a diagnosed psychopath. He was always outwardly very polite, kind, and we actually started to wonder why he was there. I would often see him whispering to himself when he was away from everyone, and once I walked by while he was doing it and he didn't notice me, and I heard him whispering about wanting to skin people alive, wear their skins, etc. in great detail. That definitely gave me the creeps hardcore.
I was not a psychiatrist but a Masters level therapist at a residential treatment center for children and adolescents. One of the most troubling clients I had was an 11 year old boy who had shot his mother to death while she was cooking dinner. He had to reload the shotgun 12 times meticulously. He also almost killed his father, who managed to stagger outside where a neighbor called 911. The boy was psychiatrically hospitalized. He showed no signs of psychosis. His older brother had told him if he killed their parents he would not have to do homework anymore. He never expressed any genuine remorse though he could state that what he did was wrong.
